Initiate a Grievance

If your contractual rights have been violated: notify your supervisor and contact the Member Resource Center (MRC) at 866.471.SEIU (7348)

Assignment Despite Objection (ADO) Form

Use this form to notify your supervisor of an unsafe assignment. The ADO form is sent to Local 1000 for tracking purposes. A copy must be given to your supervisor and a copy kept for yourself.
